My Pandemic Forms are original paintings made by photoshopping an actual Sears, Roebuck & Co. form. I then print out that form on paper that I have antiqued using Lipton Tea. The stamp is a print of an actual 1918 stamp that I have painted with a white mask over George Washington’s face. Each form is hand-painted and personalized, making each form unique. The stamp and form are held together with an antiqued paperclip. The forms are put in an envelope and mailed out though the USPS to the receiver. I do not have a specific asking price for the painted forms made for this project as they are made in support of all that have lost, sacrificed, worked, hoped, and dreamed of a more sharing future during these difficult times. I do ask, if you are able, to please support the making of this art with a contribution of $25.00 for each painted and personalized form requested. If you can pay more, it is appreciated. This painted vaccine theme is Life Savers Candy.
